Direct-to-consumer (DTC) genetic testing is a type of genetic test that is accessible directly to the consumer without having to go through a health care professional. Usually, to obtain a genetic test, health care professionals (such as doctors) acquire their patient's permission and then order the desired test. DTC genetic tests, however, allow consumers to bypass this process and order DNA tests themselves. Direct-to-consumer testing for BRCA mutations, as well as for mutations in other genes associated with various cancers, is currently available to individuals. As part of ancestry information, some companies provide BRCA gene mutation status based on microarray single nucleotide polymorphism (SNP) testing. However, this type of direct-to-consumer testing has not been validated for clinical use and generally does not provide genetic risk assessment. The NCCN does not recommend changes to a patient’s medical management based solely on direct-to-consumer testing results. The Food and Drug Administration (FDA) recommends that direct-to-consumer BRCA test results should not guide treatment; patients should have genetic counseling and confirmatory testing before any treatment decision is made. The three specific BRCA1/BRCA2 breast cancer gene mutations are most common in people with Ashkenazi (Eastern European) Jewish ancestry, accounting for more than 90 percent of their risk of heritable breast and ovarian cancer. But even in this population, only 2 percent of women carry any of these three so-called founder mutations. Women with one of the mutations have a 45 percent to 85 percent chance of developing breast cancer by age 70. In 2018, the U.S. FDA gave 23andMe the go-ahead to sell DNA tests assessing customers risk of 10 diseases, including Parkinson’s and late-onset Alzheimers. Those were the first FDA-approved direct-to-consumer tests for genetic risk of any disease or condition, which the agency said could help people make lifestyle decisions. At the time, the FDA also warned that genetic risk for complex diseases is not a sure thing. The BRCA test is a part of 23andme’s $199 Health + Ancestry test. Existing customers can opt to receive BRCA information for no additional cost, and as with the other disease-risk tests they as well as new customers must specifically choose to receive the information. In 2018, the global Direct-to-consumer Genetic Testing market size was 1078.3 million US$ and it is expected to reach 2998.3 million US$ by the end of 2025, with a CAGR of 15.7% during 2021-2026.
